Good design can make a huge difference to your website. When people see a site that looks prefessionally designed then it gives them an impression that the organisation associated with the site is also professional.
Although content is ultimately king a good design can compliment your content no end. Sometimes content is focused on so much design can be neglected. Often people can find what they need at various places on the internet. Why choose your site then? A good design which is pleasant on the eye is going to get more hits than a site which is hard to navigate and does not look good.
If people find a site which is useful to them and use it alot then a change in design can put people off or confuse them. Good design however can minimise the impact of change.
